• Communicates goals, objectives and interventions with multi-disciplinary team in order to develop client treatment plans by attending staff meetings and client care reviews.
• Documents all client activity in the medical record in accordance with program policy and procedure.
• Assists in the assessment of referrals to the program by attending meetings internally and externally.
• Performs related duties as assigned.
